Test Reservation
Form
Complete this form and return it to the address on the form. Don't forget to enclose your $35 reservation fee. Sessions are limited to 10 dogs, and they fill up quickly.
If unknown, we will contact you with the practice and test location.
Animal
Certificate of Health
Your vet must complete this certificate before you can test your dog. IMPORTANT - If we have not received this completed certificate by 5:00 p.m. on the Thursday prior to your practice session, your dog cannot participate in the certification sessions.
