Test Reservation
Form
Complete this form and return it to the address on the form. Don't forget to enclose your $35 reservation fee. Sessions are limited to 12 dogs, and they fill-up quickly.
If unknown, we will contact you with the practice and test location.
Animal
Certificate of Health
Your vet must complete this certificate before you can test your dog. IMPORTANT - If we have not received this completed certificate by 5:00 p.m. on the Thursday prior to a practice session, your dog is not eligible to participate in the certification sessions.
