Downloads for Future Therapy Dog Teams

• Reservation Form for Therapy Dog Assessment
  To sign up for a Therapy Dog Assessment, complete this form and e-mail, fax, or mail it to the Canine Therapy Corps office.  Don't forget to enclose your $50 reservation fee AND volunteer application form (see above) to reserve your space on the waiting list for a future test session. Sessions are limited to 10 dogs, and they fill up quickly (first come, first served).  Check our homepage for upcoming test dates.

• Animal Certificate of Health
  Your vet must complete this certificate before you can test your dog. IMPORTANT - If we have not received this completed certificate by the Monday prior to a Therapy Dog Assessment, your dog is not eligible to participate in the certification session.
